As an organization, we are seeking an individual that embraces person-centered planning and a collaborative process for the individuals we serve! You will work with the Development Leadership team to set and drive the organizational vision, mission, and corporate strategies.

The selected candidate will utilize data-driven analysis to measure and evaluate the performance of programs, assess needs, and ensure that program objectives are achieved. With parents/guardians and BCBS, you will ensure that programs operate efficiently, pursuing opportunities to expand and develop new programs for the individual served.

PRIMARY RESPONSIBILITIES INCLUDE:

Maintain a current understanding of the licensing requirements sufficient to ensure compliance with 245D licensing rules and regulations.

Ensure implementation and data tracking for the client’s person-centered support plans, Coordinated Service and Support Plan (CSSP), Coordinated Service and Support Plan Addendum (CSSPa), Self-Management Assessment (SMA), and Individual Abuse Prevention Plan (IAPP).

Maintain and update files and signatures required by 245D licensing rules and regulations.

Writing of quarterly and annual reports as needed or directed by the person served and/or team to fulfill person-centered plan. Attendance at monthly team meetings, and staff meetings as presented by DCs.

Creatively solve problems both individually and with a team.

Able to collaborate between various care teams and organizational departments.

Coordinate and ensure the protection from abuse and general well-being of clients.

Oversight of staff orientation, training, and supervision.

Communicate clearly and effectively with guardians, care team members, and other service providers.

Conduct performance reviews and assist with performance development.

Be available to receive calls during and outside of work hours from staff with questions or during emergencies.

Work Directly in collaboration with our team onsite and other Designated Coordinators.

Have experience working with non-verbal males and de-escalation resources (non-EUMR techniques).

QUALIFICATIONS:

Required – Must meet Designated Coordinator qualifications in accordance with the 245D requirements by one of the following:

4-year degree in a human service field and a minimum of 1 year direct care experience

2-year degree in a human service field and a minimum of 2 years direct care experience

Diploma/certificate from Post-Secondary School in a human service field and a minimum of 3 years direct care experience

Minimum of 4 years direct care experience under the supervision of a Designated Manager

Minimum of 3-years of progressively responsible supervisory and personnel management experience in Human Services.

PREFERRED

Ability to develop, implement and evaluate client programming

Ability to manage people with different learning styles

Ability to keep a healthy, welcoming work culture for all people working for/with Learnability.

Ability to problem solve and conduct scheduling assessments for team members

Knowledge of states and federal requirements as they pertain to the 245D regulations

Strong written and oral skills

Experience with budgeting/financial management

Full-time employees receive a bank of PTO/ESST after 90 days of employment (160 hours/20 days yearly).
