Responsibilities
Responsible for the completion of intakes with person-centered served plan with team members.
Assist with developing/implementing varieties of client’s person-centered support plans.
Responsible for maintaining all signatures, updating all files and all compliance matters as required by COMPREHENSIVE HOME CARE WITH HOME & COMMUNITY-BASED SERVICE DESIGNATION licensing rules and regulations.
Assist with writing periodic reports as needed or directed.
Should have good communication, interpersonal and conflict management skills.
Expected to be available to receive calls after hours from team members and clients or during emergencies.
