Case Manager

Summit Medical Consultants is recruiting for a Case Manager that will support our practice with clinical administrative assistance. This is a full time hourly position, with a full benefits package

Summit Medical Consultants is a rapidly growing Physician Practice. Founded in 2015 with 3 employees, we have expanded to approximately 75 employees and are continuing to grow. If we were a hospital in Colorado, we would rank 9th by number of admissions per year. Our mission is to provide patients and their families with robust clinical services throughout their Acute Inpatient Rehab, Skilled Nursing, Long Term Care and Assisted and Independent Living stays. We coordinate with the Hospitalists and Specialist physicians and surgeons, Physical, Occupational, and Speech Therapies, Case Management and Psychiatry at the facility level to provide compassionate and coordinated care.

Job responsibilities include, but are not limited to:

Assessing patient’s physical and mental wellness, needs, preferences and abilities and using these to develop tailored care plans

Working with clients, family and friend support networks and health care professionals to put care plans in place

Recording clients’ progress including charting referrals, home visits and other notable interactions

Evaluating clients’ progress periodically with clinical provider team and making adjustments as needed to improve outcomes

Coordinate discharge planning for skilled nursing patients

Manage and build relationships with Home Health and Hospice partners

Assist Practice Management team in tracking, monitoring and intervening in ACO/ISNP and Optum patient metrics

Assist provider teams in Annual Wellness Visits within our Long Term Care and Skilled nursing facilities

Track and implement improvement plans to decrease return to hospitalizations and improve overall health and wellness for our patients

Critical thinking and problem solving to determine the best plan of care after assessing patient’s chart, reviewing with clinical providers and implementing interventions

Manage and communicate barriers to Practice Management team

Transitional care management program- schedule, assist and complete TCM calls and Visits

Assist with TeleMedicine visits as assigned by Practice Manager

Maintain positive professional relationships with company vendors

Build and maintain new relationships with facilities and Home health Vendors

Training care coordinators

Administrative duties as assigned

Oversight of care coordination team

Delegate appropriate assignments to care coordinator teams MA/LPN

Participate in admin on call rotation with other admin personnel

Manage patient caseload for entire practice with support from Care Coordinators and Triage Manager

Job Qualifications:

LPN or RN active license

Minimum of 1 year Case Management or Post acute care experience

Experience in use of Electronic Medical Records

Preferred experience in ACO an/or value based care models

Strong written and verbal communication skills

Experience with Google suites and smart sheets preferred

Salary: $65,000.00 - $70,000.00 per year
