Description

Care Connected is a provider of Tele Cognitive Behavioral Therapy with plans to develop a national network of quality Tele CBT providers that achieve superior outcomes for payers and injured workers. Care Connected offers a full continuum of psychological services from early identification of injured workers at risk for delayed recovery to chronic pain management education and treatment effective in getting injured workers reengaged in daily life activities including work.

Care Connected is seeking an experienced, strategic, team and results oriented Licensed Marriage Family Therapist or Licensed Clinical Social Worker in the state of CA.

Key Duties and Responsibilities

 Develop and implement treatment plans for these patients utilizing a cognitive behavioral approach.

 Work within a multidisciplinary team consisting of pain physicians, psychologists, physical therapists and other wellness professionals to ensure the provision of high quality, patient centered clinical care and optimal, consistent, predictable outcomes.

 Follow the treatment protocols and reporting processes as established by Care Connected and presented to you during the Care Connected training program.

 Lead individual Tele-CBT therapy sessions designed to assist patients with their functional and emotional recovery.

 Support and promote compliance with the Company’s policies and procedures, as well as federal, state and local laws, regulations and requirements.

 Identify and implement best practices, process improvements; establish and monitor key performance metrics.

 Practice in ways that meet all appropriate ethical, professional, and legal standards.

 Maintain current licensure and participate in continuing education as necessary to support the ongoing practice.

 Maintain confidentiality of patient records and information. Use technology in ways that are consistent with ethical and responsible professional practice.

 Establish and maintain expertise in using technology such as data management, report writing, web pages, literature reviews, and data analysis.

Qualifications

 Licensed Marriage Family Therapist

 Licensed Clinical Social Worker

Knowledge / Skills / Competencies

 Knowledge of outcomes measurement of clinical quality and utilization.

 Knowledge of CBT and chronic pain treatment.

 Knowledge of state workers’ compensation laws, regulations and standards a strong plus.

 Ability to work with and direct staff

 Ability to form strong internal and external relationships.

 Tenacity and attention to detail in working with interdisciplinary teams.

 Ability to analyze complex situations, collect data, establish facts, draw valid and substantiated conclusions.

 Excellent written and oral communication skills.

 Ability to react decisively and quickly.

 Ability to organize and prioritize

This is for a part time position for approximately 10 hours a week on days/times designated by the clinician. Bilingual is a plus. There is opportunity for growth.
