VCC is an equal opportunity employer.ResponsibilitiesProvide case management, home or field visits, coordinate services for program participants, and assist in program activities and administrative support. Act as lead to support Case Managers with job duties.Process Program referrals and assign clients to Case Managers based off caseload report.Review caseload report monthly and report finding to Program Supervisor/ManagerAssist with training of new Case ManagersAct as a resources for Case Managers on community referrals, updating referral list as neededPresent positive role modelEnroll participants based on program eligibility criteriaDocument case management using the program templates (Service Plan, Case Notes, Referral Log, Contact Log, Consent Forms, and other forms as directed)Monitor the coordination of services and documents additional needs identified for each participantMonitor utilization of services and careEncourage and monitor that participants complete the educational sessions QualificationsMinimumHigh school graduate or equivalent with seven years case management experience, or equivalent combination of education and experience; Valid California driver’s license, current vehicle insurance, and reliable transportationBilingual English/SpanishAvailability and willingness to work evening and weekend hours, as requiredPreferredBachelor's degree in related field Required Skills/Knowledge/AbilitiesAbility to present information effectively verbally and in writing to individuals and groups from various cultural, ethnic and educational backgroundsAbility to work independently and under time constraintsAbility to handle confidential materials and information in a professional mannerExcellent customer service skills and commitment to providing the highest level of customer satisfactionExperience/familiarity with computers, business e-mail, communication systems and internet search capabilities and proficient in Microsoft Office products, specifically Word and ExcelPay Range$21.00 to $25.00 DOE PI241114609 Apply
