Rehabilitation Care Coordination is a woman owned small business; we are a private nurse life care planning & care management company based in San Diego. We have been serving patients with catastrophic injuries throughout Southern California for over forty years. The company specializes in comprehensive Case Management and Life Care Planning including coordination of services, cost containment, discharge planning, integration of services and assessment of short- and long-term goals
We are looking for a full-time on-site/in-office Social Worker/Case Manager for an at-will position. Our ideal candidate is self-motivated and an energetic self-starter who is a strong advocate that can collaborate to help our patients achieve their goals towards independence.
Job Duties and Essential Functions of the Position
Assess what benefits each patient may need to create a plan of action
Complete all necessary applications and processes to secure community, state & federal benefits for our patients
Educate patients about resources and options for their benefits and care
Advocate for the patient at all hearings
Liaison between patients, their families, the Case Management team on all benefit matters
Participate in all discharge and long-term care planning
Ensure compliance with all regulatory bodies
This position primarily involves sitting for extended periods at a desk while using a computer, phone, and other office equipment with occasional standing, walking and reaching.
Travel to patient appointments at a variety of locations throughout San Diego County is a requirement of the position
Qualifications
Bachelor's in social work, behavioral science or any similar field.
At least 2-3 years of experience in the community.
Health care experience preferred
Knowledge of current federal and state resources
Ability to work both independently as a self-starter and as part of a larger team
Experience performing research & completing applications on the internet, as well as experience with Microsoft Office
Excellent verbal and written communication skills
Valid California Driver’s License with Auto Insurance and a reliable vehicle
Bilingual Preferred (Spanish ideally but any additional language is a positive)
Physical Requirements:
Prolonged periods of sitting at a desk and working on a computer and prolonged sitting while driving.
Must be able to lift 15 pounds at times.
Must be able to access and navigate each department at the organization’s facilities.
Benefits
Hourly wage between $35-45/hr based on experience, this position is non-exempt.
