VCC is an equal opportunity employer.ResponsibilitiesProvide HIV medical case management to program participants and assist in program activities and administrative support.Enroll clients into the programProvide intensive case management assessment and care coordination to clientsMonitor participant’s utilization of care with VCC and other agenciesCoordinate appointments between clients and staffQualificationsMinimumHigh school graduate or equivalentFive years' case management experience or equivalent combination of education and experienceBilingual English/SpanishValid California driver's license and vehicle insurance; reliable transportation, to travel for meetings and/or to gather information for reporting purposes; minimum two years' driving experience; no more than 2 violations or a single ticket/accident value at more than 1 point on driving recordAvailability and willingness to work evening and weekend hours, as requiredRequired Skills/Knowledge/AbilitiesKnowledge of co-occuring conditionsEffective written/verbal communication skillsFamiliarity with local HIV/AIDS and non-HIV specific resources for health and social service needsFamiliarity with health and social service needs of target populations servedBasic computer knowledge, internet use, MS Office and data entry skillsPay Range$22.00 - $24.00PI275026165 Apply
