The ideal candidate will possess strong clinical skills, excellent communication abilities, and a dedication to providing high-quality, patient-centered care. This role requires a proactive approach to managing patient health, coordinating care across multiple disciplines, and advocating for optimal patient outcomes.Responsibilities include:Conduct comprehensive assessments of patients' physical, psychosocial, and emotional needs. Develop and implement individualized care plans based on assessment findings and in collaboration with the interdisciplinary team. Coordinate care with physicians, specialists, hospitals, home health agencies, and other healthcare providers. Monitor patient progress and make necessary adjustments to the care plan as needed. Educate patients and their families about their health conditions, treatment plans, and self-management strategies. Advocate for patients' rights and needs.Manage transitions of care, ensuring smooth transitions between healthcare settings. Utilize electronic health records (EHR) to document patient encounters, care plans, and progress notes. Participate in case conferences and interdisciplinary team meetings. Identify and address potential barriers to care. Track key performance indicators (KPIs) related to patient outcomes and care coordination. Maintain current knowledge of healthcare trends, regulations, and best practices.Qualifications

Current Registered Nurse (RN) license in Washington.

Graduation from an accredited school of nursing approved by the Washington State Board of Nursing.

Strong clinical assessment and critical thinking skills.

Excellent communication, interpersonal, and organizational skills.

Ability to work independently and as part of a team.

Proficiency in electronic health records (EHR) systems.

Knowledge of healthcare regulations and reimbursement methodologies preferred.

Preferred:

Bachelors of Science of Nursing strongly preferred.

Experience managing complex patients or patients with chronic disease in an ambulatory setting.

Certification in Specialty area such as:

American Academy of Ambulatory Nursing in Certification in Care Coordination and Transition Management (CCCTM).
