POSITION TITLE: CASE MANAGER

MISSION: To disrupt the cycle of homelessness and promote sustained independence for current and future generations.

SUPPORTED BY: Program Manager, Housing Program Director

SALARY RANGE: $24.61-$29.66/hour, 40 hours/week, non-exempt

Candidate offers consider license, experience, qualifications, and internal equity.

Entry salary is typically in the first third of the range.

BENEFITS PACKAGE: Paid time off for this position includes up to 4 weekly wellness hours, 19 days of PTO in the first year of employment (PTO includes sick time), and 12 paid holidays per year. Other benefits include agency-sponsored medical/dental insurance (current premium covered at rate of 100% for the employee), Employee Assistance Program (EAP), premium Calm meditation app subscription, and employer retirement plan contribution (current rate of 3%) after first year of employment. POSITION SUMMARY: The Case Manager provides housing case management to individuals and families who reside at the Domestic Violence and Sexual Assault Services (DVSAS) safe shelters, along with periodic housing consultation and support to residents at Engedi Refuge. The Case Manager position helps survivors fleeing domestic violence in accessing Coordinated Entry and assisting families and individuals with identifying and working toward housing goals to meet their needs, advocacy, problem-solving and accessing community resources.

MINIMUM QUALIFICATIONS:

Demonstrated experience equivalent to a Bachelor’s degree in Social Work, Human Services or related field AND 1 year of related experience in direct service, or equivalent combination of education and experience.

Basic knowledge of housing and support resources for low-income families in Whatcom County

Ability to establish rapport and build on family strengths in the home environment

Experience working with diverse groups of people

Proficiency with word processing, Microsoft Office programs, and cloud-based internet software

Completion of background investigation that includes social security verification, professional reference checks, and a federal criminal background investigation.

Valid driver’s license, access to a vehicle and proof of insurance required

Proficiency in Spanish, Russian, or Sign Language preferred

Due to Washington State mandate, all Lydia Place employees and volunteers are required to have full COVID-19 vaccination and ability to furnish proof of such by first day of work for continued employment. Exceptions for individuals with a medical reason or bone fide religious belief will be reviewed and determined by Lydia Place Directors.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

DVSAS PARTNERSHIP (.95 FTE):

Evaluate and assess each household who enters the DVSAS family emergency domestic violence shelter to determine housing needs, program eligibility, and the most effective and efficient path out of homelessness with the goal that all households have a program placement within 90 days

Conduct and intake and SPDAT assessment for the housing interest pool and ensure that eligible households are entered into the Coordinated Entry System

Provide interim case management for shelter residents until they are enrolled in a permanent or other housing program, or until an alternate housing option has been obtained

Act as a liaison to Coordinated Entry Housing Pool to advocate for housing placements including maintaining weekly communication with Coordinated Entry

Coordinate with DVSAS safe shelter advocates regarding housing goals, supports, and other household needs

Compile and submit monthly report of client service summaries to DVSAS

Make referrals as needed for DVSAS’ most vulnerable households to receive Section 8 vouchers under our BHA Referral Voucher Program partnership and support with the application process, lease-up, and provide ongoing housing case management for DVSAS clients who receive a Section 8 voucher, maintaining an estimated caseload of 5 households in permanent housing with these vouchers

Coordinate with DVSAS advocates to provide housing consultation to clients not residing in the safe shelters but in need of housing guidance, advocacy, and support

Collaborate with Lydia Place Parent Support Program, Mental Health team, and other staff to identify and offer additional programs and support services to meet client needs

ENGEDI PARTNERSHIP (.5 FTE):

Evaluate and assess each household at Engedi who wishes to remain in Whatcom County to determine housing needs, program eligibility, and the most effective and efficient path out of homelessness. If applicable, complete intakes for such households to access Coordinated Entry.

Provide renter’s education and housing consultation support to other Engedi residents as requested.

COMMUNICATION, DOCUMENTATION, OTHER ESSENTIAL FUNCTIONS:

Maintain accurate and timely documentation and submission of client files and funding/contract paperwork

Enter data into both AGENCY and HMIS, including compliance with all contract and confidentiality requirements

Fulfill all duties as a mandated reporter of child abuse or child neglect to DCYF

Maintain client confidentiality in all circumstances expect those required by law

Participate in weekly case management supervision and Team Meetings

Communicate effectively with clients, colleagues, supervisors, and community partners

Represent Lydia Place at designated community meetings and through public presentations as assigned; preserve positive professional relationships with service providers in the community

Participates in the on-call rotation
