Adult Case Manager, Crisis Intervention and Stabilization TeamPOSITION SUMMARY:The Case Manager assists individuals with obtaining access to, and coordinating health care, behavioral health treatment, social services, advocacy, educational programs, recreational programs, and community resources. The Case Manager also identifies and monitors the need for services tailored to the individual and delivered in the community and explores available resources.DUTIES AND RESPONSIBILITIES:

Work in partnership with the treatment team, with participation from natural supports, in the development of the resiliency plan (the intake assessment, functional behavior assessment (FBA)) and evaluation(s) which includes the identification of health care, behavioral health treatment, social services, advocacy, educational, recreational, and community resources to prevent further escalation/decompensation of the individual; link and coordinate services for the individual based on the resiliency plan objectives

Follow up with the appropriate organizations such as healthcare, behavioral health, and social services agencies, with activities including but not limited to in person interactions, and email communications to make sure that services and resources are in place for the individual that were identified during the development of the resiliency plan

Provide updates, regarding client goals and the client's overall well-being, to the interagency team during meetings and service planning, sharing the resources that have been found for the families, identifying any additional family needs to ensure continuity of care and overall natural support system stability

Follow up and/or obtain any documentation that is missing or needs updating for the file to include the caseload chart, intake assessment, FBA, evaluation, authorization packet, and progress notes based on regulatory standards with the Department of Behavioral Health and Disability Services (DBHIDS) and Community Behavioral Health (CBH) to ensure program compliance

Assist the individual to prevent the need for restrictive, acute care services by continuing to monitor services, including but not limited to meeting with the individual for periodic check-ins, and connecting them with additional resources in the community, as needed, to make sure the needs of the individual are met

Perform cardiopulmonary resuscitation (CPR), restrictive procedures and manual restraints using agency-trained protocols

Bachelors' degree or above in a behavioral health or related human service field

Experience working directly in behavioral health preferred

Knowledge of MCOs is a plus

Demonstrated ability to work effectively as part of a team

Ability to work in a fast-paced environment and manage and prioritize multiple, often competing, priorities

Demonstrated strong attention to detail

Demonstrated strong time management and organizational skills

Demonstrated excellent judgment with the ability to independently solve problems and make decisions with little or no need for direct supervision

Demonstrated ability to anticipate future needed changes or identify problem areas and take effective actions

Must be able to perform cardiopulmonary resuscitation (CPR), and restrictive procedures and manual restraints using agency-trained protocols

Must possess excellent customer interaction, collaboration, presentation, and written and verbal communication skills

Demonstrated computer proficiency, especially with Microsoft Office Suite (Microsoft Word, Excel, PowerPoint and Outlook)

Knowledge of Electronic Health Record (EHR) / Electronic Medical Record (EMR) preferred
