Paradigm is seeking a licensed Behavioral Health Consultant to assist our client in providing quality mental health services to a diverse population of patients. This is a good opportunity for someone looking to provide direct positive impact on the community they are a part of.

Type: Direct Hire, Full time

Location: Salem, OR

As a Behavioral Health Consultant, you will:

Collaborate with and assist medical providers and other health care team members in recognizing and treating emotional, behavioral, and mental health disorders

Provide goal-oriented consultations and interventions to address mental health, behavioral health, and co-morbid physical conditions

Assess at-risk patients and provide appropriate intervention services

Provide and support patient education

Provide short-term problem-focused therapeutic modalities based on clinic need. Services are generally provided in 20-40 minute sessions

Facilitate referral and coordinate with Care Management team to address patient social determinants of health needs

Attend and participate in ongoing monthly meetings as needed

Provide psychiatric diagnostic evaluations based on primary care team needs

Accurately record psychological and behavioral health histories utilizing established protocols and EHS

Work to deescalate potential crisis situations

Perform risk assessments, and collaborate with patients support team

Provide mental health assessments, including assessing mental health and/or behavioral symptoms using a variety of standard assessment tools, develop individualized treatment plans, develop crisis prevention plans, and direct provisions of treatment services

Perform other related duties as assigned

Requirements:

Master’s or Doctoral degree in a specific professional field related to the position and an active Oregon license, or possess the ability to obtain an Oregon license (PsyD/PhD/LCSW/LPC)

Bilingual in English & Spanish preferred

Experience and interest in women’s health

Experience working with EHR

Experience with chronic disease

Experience working with diverse populations

Previous experience working in a primary care setting or community mental health

Knowledge of program development & assessment
