Job Title: Resource Referral Specialist
Reports To: Program Director
FLSA Status: Hourly
Hours: 32 Hours/Week
Office Location: Multnomah Village/Remote
Current Pay Rate: $19-$21 Starting

For over 30 years, Store to Door has delivered nourishment and social connections to homebound people in our community. We support independent living for Portland area seniors and people living with disabilities by providing a low-cost, volunteer-based grocery shopping and delivery service. Our award-winning organization is the only one of its kind in Oregon. Each year, with over 1,400 volunteers, we make over 14,000 deliveries to 800+ seniors and people living with disabilities.
Position Summary:
The Resource Referral Specialist provides case management and referrals to services and community resources to clients enrolled with Store to Door. This includes creating an efficient system for maintaining a case load of up to 25 clients, maintaining client files, performing screenings with clients, providing Information & Referral services to clients referred to the program, and coordinating volunteers within these programs. Responsible for database entry, case notes, and referral tracking. Coordinate work with the program, volunteer, development, and office teams to maintain effectiveness. Provide support for the Hollywood and Beaverton stores as assigned.
Duties & Responsibilities
Closed Loop Referrals Duties (100%)
Receive requests for services from clients, community partners, and colleagues at Store to Door
Provide referrals to services and supports to clients and provide short-term case management to clients with complex needs
Collaborate with other members of Program Team to provide timely closed loop referral to clients
Complete needs screenings with clients currently and newly enrolled at Store to Door
Coordinate volunteers within Resource Referral Program
Coordinate with community partners and community members to ensure that clients’ needs are met
Report any alleged abuse of clients to appropriate services and agencies
Record case notes and service referrals in both UniteUs and FileMaker databases in a timely manner
Participate in the Resource Referral Team, Program Team Meeting, and All Staff Meeting as scheduled
Assist in data collection, special projects and programs, and generate reports as needed
Maintain current information on available community resources
Ensures all client services are inclusive, responsive, and collaborative
Other duties relevant to this position and Program Team operations
Qualifications
Bachelor’s Degree, Masters in Social Work or related field preferred
Demonstrated experience with client services required
Experience working in Diversity, Equity, and Inclusion.
Skills, Abilities & Experience:
Minimum of two-years’ experience providing case management, resources and referrals to clients
Ability to promote a culture of respect, inclusiveness, equity, and diversity
Ability to present ideas in both business-friendly and user-friendly language
Ability to vary work hours to accommodate occasional weekend/evening hours
Knowledge of and passion for working with seniors and people with disabilities
Excellent verbal, written, public speaking, and interpersonal skills
Excellent organizational and time management skills, including the ability to set priorities and organize multiple projects. Must be flexible to changing deadlines and last-minute work assignments
Strong written and oral communications skills
Ability to work independently, with a team, and communicate with diverse populations
Ability to meet deadlines and work well under pressure with attention to detail
Proven analytical, evaluative, and problem-solving abilities
Must maintain confidentiality at all times
Candidate must successfully pass a criminal background check
Any satisfactory combination of experience, training, and/or education that ensures the ability to perform the work may substitute for the above qualifications.
Working Conditions/Physical Demands
Fast-paced, with significant people interaction. No lifting over 20 lbs. Some reaching, bending, and stooping, and sitting for extended periods with phone and computer work.
