Are you looking for a job that will make a difference in your Community? Benton County Behavioral Health Early Assessment and Support Alliance Team is currently looking for a Bilingual Health Navigation Coordinator.

Introduction to Early Assessment and Support Alliance (EASA)

EASA is an innovative program for youth and young adults (ages 15-25) who have recently begun experiencing unusual thoughts and behaviors or who have started hearing or seeing things that others don’t. EASA provides outreach throughout Benton County towards rapidly identifying individuals needing early intervention to prevent the issues associated with untreated psychosis. EASA collaborates with families and community partners to build strong support networks.

EASA provides expert assessment as well as individualized treatment planning. For those meeting criteria for enrollment, EASA delivers approximately two years of coordinated specialty care services including: counseling, case management, skills training, psychiatry & nursing, health education, family supports, peer supports, employment & education supports, social groups and much more. EASA services are transitional: the focus is on supporting individuals and their families to develop the knowledge, skills, and resources to be successful in the long run.

Job Summary

The EASA Bilingual Clinical Health Navigator will coordinate the activities of EASA Community Education to reach diverse communities in Benton County, with a particular focus on serving Latino & Hispanic community members.

The EASA Bilingual Clinical Health Navigator will be responsible for facilitating development of EASA referral and retention networks between diverse communities and EASA. This will be achieved through outreach and engagement to these communities. Methods of outreach will include individual information sessions, group workshops, creating and maintaining networks with other agencies and organizations. This position will also be responsible for data gathering, reporting, and monitoring.

Please note:

This is a regular status, part time, benefit eligible position at .5 FTE (20 hours a week).

This is a Bilingual Spanish/ English position and offers a Bilingual Differential of base salary plus an additional 5% upon successful completion of required language testing.

Salary range of:

$21.82 to $28.63 per hour

This position guarantees two salary raises within your first year as long as your performance is in good standing!

Minimum Qualifications

The following minimum qualifications are required for this position:

Associate's Degree in Public Health, Education, Communications or a closely related field; AND

2 years of related work experience in Public Health, Education, Community Outreach, Health Services or Health Navigation; AND

Bilingual skills in Spanish & English: This position is Bilingual and requires duties to be done in both English and Spanish.

An equivalent combination of education and experience may be accepted. Please note candidates must have at least 6 months of actual work experience to be considered as equivalent.

Special Requirements:

Hiring is contingent upon the successful completion of a background check.

Valid Oregon driver's license.

The Ideal Candidate

Community Health Worker Certification with the Oregon Health Authority.

Experience using the OCHIN and EPIC Electronic Health Record Systems.
