Job Title:
Coordinator of Care RN, Home Care (CHHA)
Salary: $100,000 - $119,600 based on the following differentials: education, experience, nursing certificates and bilingual skills (Spanish, Cantonese, or Mandarin)
$15,000 Sign-on Bonus or Student Loans Assistance, paid out over the course of 3 years
Location: Car and driver license required for Brooklyn, Queens, and Bronx regions
Hours: Monday-Friday 9am-5pm
About the Role:
Perform a comprehensive physical, psychosocial and safety assessment via examination, observation and motivational interviewing.
Perform a comprehensive medication reconciliation the includes all physician prescribed and OTC medications and/or supplements.
Verify the initial plan of care with the patient’s physician; utilizing patient specific parameters inform the physician of any changes in the patients’ status; update the plan of care to reflect changes made by the physician.
Assess the patient for the appropriate level of care and utilization of agency resources.
Utilize motivational interviewing to engage patient/caregiver in the established plan of care.
Ensure the EMR accurately reflects: the current plan of care; the services provided; patient’s response to treatment; status of goal achievement.
Effectively communicate and coordinate the delivery of services across all disciplines, including physicians, PSM, WOCN, CDE, and MLTCP.
Accurately complete all OASIS data sets, within agency established guidelines, which reflect positive clinical outcomes and consistent utilization of industry established best practices.
Accurately complete all OASIS data sets to ensure the correct reimbursement is received.
Data enters into the EMR all physician orders on the same day the order is received from the physician.
Provide accurate and timely documentation requesting authorization of services from MCOs in a manner that supports continuity of patient care and reimbursement for services provided.
Submit all split bills and payer changes accurately and timely to finance; respond to all inquiries from finance requesting clarification of information submitted.
Minimize the occurrence of NTUCs, LUPAs, Outliers.
Consistently adhere to all agency guidelines as well as all state and federal regulations specific to the delivery of home care services.
Demonstrate expert knowledge related to scoring of all OASIS data set items.
Qualifications:
Walking or moving within and outside workplace
Kneeling or crouching when providing home visits
o Lifting > 50 lbs when providing home visits
o Performing patient care techniques which require manual dexterity, visual inspection, manual palpation, auditory and visual discrimination
o Ability to negotiate architectural barriers.
o Associates Diploma in Nursing, required, Baccalaureate Degree, highly preferred.
o Minimum of one-year acute, Medical-Surgical nursing experience; CHHA/LTHHCP experience preferred.
o Knowledge of Microsoft Office and experience documenting in a clinical software system preferred. Driver’s License highly preferred.
