Req Number 134026Job DescriptionProvides guidance and participates in the development, implementation and evaluation of diversity and health equity initiatives across the organization. Builds community relationships to support partnerships between the organization and the community to support diverse outreach initiatives.Job Responsibility

Serves as a liaison to system wide departments requiring educational tools; collaborates with management to create a vision and plan for health literacy activities throughout the organization.

May serve as project/technical lead for ongoing process or short-term project within community outreach function.Provides stake holders with all necessary documentation and information; explores additional programs and training and makes recommendations based on findings.

Monitors inventory of patient education materials.

Provides periodic updates, reports, and makes recommendations to management regarding: Health Literacy services in meeting the needs of patients, health care providers, and the community.

Identifies areas for possible improvements regarding health literacy services in the community and addresses problems within scope of responsibility.

Provides resources and educates staff on health literacy in an effort to deliver services that align with the internal and government regulations.

Conducts meetings disseminates information, collects data, writes reports, and seeks expansion of diversity and health equity initiatives across the organization.

Works independently under minimal supervision within established guidelines and procedures.

Job Qualification

Bachelors degree required, or equivalent combination of education and related experience.

3-5 years of relevant experience, required.
