Outpatient Case Manager

Work hours are 8-430pm, Mon thru Friday.

Case Manager will shadow groups with PPJ therapist.

Case Manager will complete the Education Group for PPJ M-F1-230pm.

Case Manager will work with all outside and inside referrals to place into the correct PHP and IOP program.

Case Manager will update, identify discharges, psychiatrist appointments, and update the Outpatient Program Boards.

Case Manager will communicate with all departments in order to ensure proper admissions.

Case Manager will manage outside referrals, placement, and communication with outside referrals.

Case Manager will be trained on all therapist information as well as case management duties.

Case Manager will review duties for both PHP and IOP programs.

Case Manager will eventually work on discharge plans and referrals after completing the Outpatient Programs.

KEY RESPONSIBILITIES:

Develops and coordinates an individualized discharge plan for the patient by utilizing treatment team and written chart information to determine the patient's aftercare needs.

Accesses by phone or written correspondence family or community resources such as nursing homes, group homes, or private mental health practitioners to meet patients' aftercare needs.

Master's level Case Manager provides individual, group, and family psychoeducation-oriented care using various professional treatment modalities.

Coordinates individualized discharge planning for patients with physician.

Documents communication in progress notes regarding patient in medical record.

Attends treatment team meetings with multidisciplinary team.

Reports incidents of suspected abuse using prescribed forms and protocols from the relevant community/state agency.

Maintains effective working relationships and communications with internal and external customers.

Schedules, attends, and documents court hearings as required.

Requirements

Bachelor’s Degree in Psychology or related field required; Master's degree preferred.

Minimum three (3) years behavioral health experience or combination of education and experience preferred.

Familiarity with medical and psychological terminology and human behavior principles

Experience with therapeutic limit-setting or an understanding of level systems or other behavior management modalities helpful

Ability to maintain professional boundaries

Maintains confidentiality of patients at all times
