Hiring for Care Management Support Coordinator (100% Remote Position) Missouri, MO, USA Candidates onlyDescription:Position Purpose:Provide essential administrative support for care management, including outreach, call handling, scheduling, and documentation. Act as a key contact for members, providers, and staff to resolve issues and ensure compliance with state and regulatory guidelines.Education/Experience:
High School diploma or GED
1-2 years of related experience in a fast-paced call center
2+ years of healthcare experience and data entry
Key Responsibilities:
Conduct member outreach and support care plan activities.
Answer inbound calls and schedule services.
Connect members with health plan and community resources.
Document and maintain non-clinical member records.
Support member onboarding and manage correspondence.
Explain care plan procedures and protocols.
Required Skills:
Strong communication and problem-solving skills
Proficiency in MS Office (Excel, Word)
Ability to adapt to change and handle a high-volume environment
Preferred Skills:
Familiarity with medical terminology
Typing
