Join UnitedHealth Group and our family of businesses and you will use your diverse knowledge and experience to make health care work better for our patients.The Care Coordinator (Social Worker) is responsible for updating care plans, completing daily case notes, and assigning outbound referrals and / or task to close care plan items. Daily responsibilities includes but not limited to making outbound calls to enrolled members, review, research and respond to questions or issues via email or phone, care plan documentation, reviewing reports, identifying community resources by keeping up with updates and changes in the industry while working virtually from home. This role will require you to have good time - management skills, be self - directed with minimal supervision, self - motivated, organized, and detailed oriented.To ensure success in your role, performance expectations measured are quality (gaps / barriers addressed within 90 days), growth (engagements resulting in enrollment), people / employee recognition (employee retention), satisfaction (NPS satisfaction survey), and finance (monthly enrollment aligned with budget).This position is full-time. Employees are required to have flexibility to work any of our 8-hour shift schedules during our normal business hours of 7:00am – 7:00pm CST. It may be necessary, given the business need, to work occasional overtime.We offer up to 6 weeks of paid training where you will be paired with a peer and / or leader. The hours of the classroom & peer training will be 8:00 AM - 5:00 PM CST from Monday - Friday. Training will be conducted virtually from your home.You’ll enjoy the flexibility to work remotely from anywhere within the U.S. as you take on some tough challenges.Primary Responsibilities:

Assess, plan and implement care strategies that are individualized by patient and directed toward the most appropriate, lease restrictive level of care

Identify and initiate referrals for social service programs; including financial, psychosocial, community and state supportive services

Manage the care plan throughout the continuum of care as a single point of contact

Communicate with all stakeholders the required health - related information to ensure quality coordinated care and services are provided expeditiously to all members

Advocate for patients and families as needed to ensure the patient's needs and choices are fully represented and supported by the health care team

Generally work is self - directed and not prescribed

Works with less structured, more complex issues

Serves as a resource to others

Assesses and interprets customer needs and requirements

Identifies solutions to non - standard requests and problems

Solves moderately complex problems and / or conducts moderately complex analyses

Works with minimal guidance; seeks guidance on only the most complex tasks

Translates concepts into practice

Provides explanations and information to others on difficult issues

Coaches, provides feedback, and guides others

Acts as a resource for others with less experience

Bachelor's degree in Social Work (BSW) OR other related field

Licensed BSW with applicable state licensure (where work is performed)

2+ years of experience in a hospital, long - term care, home health, hospice, public health, OR assisted living

Experience with navigating local community healthcare and social services resources

Intermediate proficiency with using Microsoft Word (create and edit documents), Microsoft Excel (create and edit spreadsheets, filter, sort, basic formulas), Microsoft Outlook (calendar management, emails)

Ability to work any full-time, 8-hour shift during our normal business hours of 7:00 AM - 7:00 PM CST including the flexibility to work occasional overtime given the business need

Preferred Qualifications:

Master’s degree in Social Work (MSW) with License (applicable state licensure)

Experience with managing populations with complex medical OR behavioral needs

Experience with electronic charting

Experience with arranging community resources

Bilingual fluency in English and Spanish

Telecommuting Requirements:

Ability to keep all company sensitive documents secure (if applicable)

Required to have a dedicated work area established that is separated from other living areas and provides information privacy

Must live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service

Soft Skills:

Must be able to work independently and be self - directed

Good time management skills

Solid organizational, attention to detail and interpersonal skills

Ability to identify and implement appropriate patient / enrollee communication strategies and overcome accessibility barriers, as required

Ability to manage multiple responsibilities, prioritize scheduling of work and meet deadlines

Ability to maintain confidentiality of all medical, financial, and legal information

Effective communication both orally and written

Ability to handle difficult situations involving patients / enrollees, physicians, or others in a professional manner

Problem solving skills; the ability to systematically analyze problems, draw relevant conclusions and devise appropriate courses of action

Residents Only: The salary range for this role is $58,300 to $114,300 annually. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
