Job SummaryClinical Pharmacy Specialists within the Pulmonology space will work closely with providers under a Collaborative Practice Agreement (CPA) to evaluate, optimize, and counsel on medication regimens for mutual patients. They will provide guideline and evidence based Chronic Disease and Medication Therapy Management directly to patients within a clinic setting. The pharmacist will focus on assisting patients with cost, adherence, and lifestyle barriers related to their medications. In addition to assisting the patient needs, they will work to develop prescribing, formulary, and clinical guidance for costly specialty medications. They will work closely with clinic staff to inform them of guideline and medication updates and collaborate with other care team members to develop care plans. They will be expected to navigate EPIC and utilize features within the EMR daily.Coordinates educational programs and activities designed to enhance clinical drug knowledge of staff pharmacists, physicians, nurses, other care providers. Works with staff pharmacist to increase their participation in drug monitoring, pharmacokinetics and cost containment activities. Provides pharmaceutical care to patients in assigned area of practice, implement cost containment programs, provide drug information, education and clinical services which enhance the effectiveness of drug therapy and minimize patient risk. Works with multiple medication automation systems.Essential Functions

Under a Collaborative Practice Agreement with the Pulmonology providers the pharmacist will manage patients’ pulmonary chronic disease by prescribing, de-prescribing, adjusting doses, ordering, and monitoring labs.

Complete comprehensive medication reviews and work with providers to reduce the costs of medications for their patients and the system, in addition to addressing any barriers to care to help improve adherence.

Be available for any pharmacy/medication related questions for both your patients and the team.

Actively participate in any team-based care huddles and provide recommendations to assist in constructing a care plan for patients.

Review data to assist in identifying patient populations needing pharmacy interventions. Also, highlighting data on pharmacy related metrics.

Be available for questions from providers in clinic or actively participate in provider’s visit if needed/requested.

Provide education to the inter-disciplinary team on pharmacy related topics, updates on guidelines, or current events on new medications/ indications as requested.

Precept pharmacy students and pharmacy residents on site.

Evaluates patients and orders, and takes a lead role in the ordering and interpretation of drug serum levels, using pharmacokinetic principles. Provides physicians with dosage regimen recommendations to enhance therapeutic outcomes, reduce patient’s risk, and reduce the cost of drug therapy.

Provides drug information and educational services to hospital health care professionals, patients and families and assists staff pharmacists in responding to drug information requests. Serves as an information resource and clinical practice role model for staff pharmacists.

Coordinates and implements departmental drug utilization review efforts and other process improvement activities as assigned.

Ensures that processes and services are continuously monitored for quality, cost effectiveness, and efficiency. Engages in process and quality improvement activities. Makes and implements recommendations to improve operational efficiency and to implement new services for areas of responsibility.

Facilitates applicable Pharmacy Committee activities in area of specialty and participates on other committees / task forces as member, consultant and/or chair as assigned.

Engages in continuous study of the entire professional field to maintain the professional competence, knowledge, and skills necessary for satisfactory performance of all assigned responsibilities. Completes required continuous training and education, including department and Spectrum Health specific requirements on a timely basis.

Coordinates the development, editing, and dissemination of departmental publications for physicians and hospital staff.

Precepts pharmacy residents and students as assigned.

Qualifications

Required Bachelor's Degree Science of Pharmacy or Doctor of Pharmacy degree from an accredited College of Pharmacy

2 years of relevant experience pharmacy experience or an American Society of Health-System Pharmacists (ASHP) Pharmacy Practice Residency Required

2 years of relevant experience hospital pharmacy experience or an American Society of Health-System Pharmacists (ASHP) Pharmacy Practice Residency Preferred

ASHP accredited postgraduate year 2 (PGY2) residency in area of specialty Preferred

LIC-Clinical Pharmacist - STATEMI State of Michigan 90 Days required

LIC-Pharmacist Controlled Substance - STATEMI State of Michigan 90 Days required

CRT-Basic Life Support (BLS) - AHA American Heart Association Ambulatory Sites 90 Days required

CRT-Board Cert Pharmacotherapy Specialist (BC-PS) - ASHP American Society of Health-Systems Pharmacists Upon Hire preferred Or

CRT-Board Cert Pharmacotherapy Specialist (BC-PS) - BPS Board of Pharmacy Specialties Upon Hire preferred

CRT-Board Cert Pharmacotherapy Specialist (BC-ACP) - BPS Board of Ambulatory Care Pharmacist Upon Hire preferred

Ideal candidate will possess PharmD Degree and a PGY1 or PGY2 Residency in Ambulatory Pharmacy and/or experience in Adult Pulmonology

Ideal candidate will possess direct patient care experience with Cystic Fibrosis/Pulmonary Disease States

Candidates with PGY2 Training in Ambulatory Care or candidates who are Board Certified Ambulatory Care Pharmacists (BCACPs) would be highly preferred

We would welcome someone with a Community Pharmacy or Specialty Pharmacy Background if they demonstrate the ability to engage with Physicians, Insurance Payers, and Patients to Optimize Medications

This is a Monday to Friday position, 8 a.m. to 4:30 p.m., No Weekends

This is a Hybrid Position, primarily working On-Site
