Per Diem RN needed for a busy home care agency in Boston, Brocton, Brighton areas
We are looking for Haitian Creole speaking candidates but it is not requirement.
Required Education and Experience:
Must have a valid Registered Nurse's license from the Commonwealth of Massachusetts,
Must have a CPR certificate,
Must be fully vaccinated,
Must have a valid driver's license.
Essential Functions:
Conducts Home Visits as needed and clinical assessments, Initial Assessment and Re-Assessments,
Develops Care Plans that are appropriate to meet the members needs,
Completes and prepares all necessary assessment, data, form and care plans as required,
Keeps updated progress notes in accordance with EOEA documentation standards,
Works closely with, case managers, consumers, and caregivers to coordinate services.
Compensation:
Regular Visit - $60
Annual Visit - $100
