Ensure seamless coordination and exceptional quality of care, creating an environment of trust and excellence.ResponsibilitiesThe Manager, Clinical Services contributes to the organizational success of providing high quality care by ensuring the coordination, continuity and quality of clinical care following a physician’s approved plan of treatment. Facilitates the completion of documentation to comply with regulatory, operational and financial requirements. Acts as a clinical resource to internal and external customers.Responsibilities

Functions as an effective team member and contributes to the development and achievement of location and organizational goals and objectives.

Is promptly and professionally available to assist all office and clinical staff.

Demonstrates the ability to be efficient and productive by organizing job duties and responsibilities.

Supports and promotes department, branch and organization decisions.

Demonstrates sensitivity to the needs, customs, or feelings of patients and caregivers.

Participates in case conferences, summary reports, quarterly record review, annual program review and in-services or special case orientations.

Responsible for front line direction of accurate, complete, and timely provision of quality client care and services in Compliance with regulatory, operational, and financial requirements.

Manages direct reports.

Creates an “expectation of excellence” and manages performance of direct reports to maximize the value of employee’ contributions towards organizational success.

Ensures all orders are entered and approved timely for tracking purposes and to assure they are sent to physicians for signature. Ensure current orders exist for requested medical supplies. Assures missed visits reported to physicians.

Reviews and processes vital sign alert reports, document action, and any follow-up and physician notification. Reviews and processes all wound score deviations documenting any action and follow-ups.

Acts as a resource for external customers, including but not limited to, physicians, referral sources and discharge planners/social workers. Provides clinical expertise for internal customers.

Participates in office functions, including intake, payer verification, scheduling, payroll, billing resolution, and laptop support as needed and/or directed by management. Participates in designated field or management on-call rotation.

Acts as back-up to Director, Clinical Services as directed by Director, Operations.

Assists with the development of office and field processes and action plans developed as a result of clinical measurements. Assists in identifying clinical training needs.

Assists the Director, Clinical Services by delivering assigned clinical and technology systems education, training and coaching for associates in support of associate development and organizational success.

All other duties as assigned.

QualificationsCurrent Registered Nurse (RN) license (BSN preferred) or a Physical or Occupational Therapist in good standing to practice in the state serviced.Minimum of two years home health or hospice experience required.Thorough knowledge and understanding of home care regulatory requirements.Compliant with appropriate state certification and registration and organization requirements as necessary.Must obtain and maintain current CPR or BLS Certification.At VNA Health at Home, we are proud to be an Equal Opportunity Employer, promoting diversity, equity, and inclusion in every aspect of our organization.
