Schedule: Full time, exempt, varied day hours from 7:30 am - 6 pm, partially remote after 90 day on site orientation.

Connect with your Talent Advisor for additional specifics.ResponsibilitiesInvestigate, assess and plan interventions to help patients cope with social, emotional, economic and environmental problems.
Interview patients, assess priorities and documents case activity.
Consult with team managers, statutory and voluntary agencies and patient's relatives to ensure compliance with patient's treatment plan.
Assist clients/families with obtaining community assistance by referral to proper resources.
Develop and maintain working relationships with community health, welfare and social agencies and seeks creative means to assist the client with his/her needs.
Support management of concurrent review, discharge planning processes, social services, home care and other post discharge needs.
RequirementsLicensure / Certification / Registration:
One or more of the following is required:
Specialist Social Worker specializing in Clinical Practice credentialed from the Kansas Behavioral Sciences Regulatory Board obtained prior to hire date or job transfer date.
Licensed Social Worker (LSW) or Licensed Master Social Worker (LMSW) in the state of Kansas by the Kansas Behavioral Sciences Review (KSBSRB) required or must be obtained prior to hire or job transfer date.
Education:
Bachelor's degree in Social Work required.
Work Experience:
3 years of experience preferred.
Additional PreferencesPrevious social work expereince, managing patient care over the phone and navigating community resources.
