Job DescriptionWoodward Resource Center, a State of Iowa Health & Human Services (HHS) facility, is seeking a Physical Therapist 1 to join our team. This position works remotely with occasional overnight travel.About Woodward Resource Center (WRC):At WRC, our mission is to prepare and support individuals to live in the community of their choice. We’re committed to providing quality care and support tailored to each individual's needs and aspirations.What You'll Do:This position will work to improve transition outcomes, enhance services provided in the community to individuals living with intellectual disability, expand comprehensive statewide access to support services, adapt approaches to health risk management and excellence in community, and increase awareness about, public confidence in, and use of coordinated care for individuals with intellectual disability. Your responsibilities will include:Promote Excellence in Care in partnership with Community Providers.Through WRC's Center of Excellence, HHS will strengthen community providers ability to mitigate health/safety risks and improve services by:-Completing physical therapy evaluations based on study of clinical records, observation, and directassessment for individuals assigned to diagnose impairments, determine functional limitations, and disabilities and identify changes in physical function and health status.-Establishing treatment plans, based on results of evaluation and consultation with other professional and community providers, to restore health, alleviate pain, decrease the effects of impairment, enhance and improve functional abilities, diminish the effects of a disability, and enhance physical function.-Writing, reviewing, and modifying Physical Therapy (PT) maintenance programs and providing reviews of the comprehensive care team including community provider, individual, and family.-Designing and delivering competency-based training programs for staff to prevent impairment, maintain health, alleviate pain, and enhance functional ability for community participation. -Assisting individual transition to alternative placement through on-site visitation and/or phone consultation.Reduce the need for long term ICF / IID services.Through WRC's Center of Excellence, HHS will provide enhanced resources and improve service delivery by:-Engaging and offering short-term assessment, stabilization, and initial skills development necessary for individuals with intellectual disabilities to be able to return to a less restrictive integrated setting in the community.-Streamlining and leveraging current practices and programming and promoting a shared framework to promote the best possible outcomes for individuals living in Iowa with intellectual disabilities.-Recommending, designing, producing, and monitoring orthodontics and mobility equipment to advance individuals mobility, independence, and status in community living through evidence based best practice. -Expanding comprehensive statewide access to support services, adapting approaches to health risk management and excellence in community.-Increasing awareness about, public confidence in and use of coordinated care more for individuals with intellectual disabilities.Must have Licensure as a Physical Therapist by the Iowa Board of Physical and Occupational Therapy Examiners.
