Care Manager

Our client is seeking a dedicated and compassionate individual to join their team as a Care Manager. As a non-skilled personal service agency specializing in home care, their focus is on providing essential assistance to individuals in need within the Indianapolis area. The Care Manager plays a pivotal role in ensuring the delivery of high-quality care services by overseeing and coordinating a team of attendant care givers. While no formal qualifications are required for this position, a commitment to excellence in care provision is essential.
You would be the first Care Manager on the brand new team in Indianapolis. This is an in office/ in the field role, with no work from home opportunity.
What You'll Do:
Client Assessment and Service Planning: Conduct comprehensive assessments of clients' needs and preferences, and develop personalized service plans tailored to their requirements. Service plans will encompass a range of non-skilled care activities, including toileting, transferring, meal assistance, and home-making tasks such as laundry and meal preparation.
Regular Client and Caregiver Interaction: Maintain regular communication with clients and attendant care givers, conducting in-home visits once every three months to review service plans and ensure that care needs are being met satisfactorily.
Competency Evaluation: Perform competency evaluations for attendant care givers, observing their performance of essential tasks and providing feedback as necessary. Evaluations will be conducted every three months to ensure ongoing competency and adherence to service standards.
Flexibility in Schedule: Demonstrate flexibility in scheduling, as occasional evening and weekend visits to clients' homes may be required to accommodate their unique needs and schedules.
Territory Management: Manage a caseload of up to 60 attendant care givers, with the potential for growth as the agency expands. The territory will primarily encompass the Indianapolis metro area, including Marion County.
Office and Field Work: Fulfill duties both in-office and in clients' homes, balancing administrative responsibilities with direct client interactions. This is not a work from home role.
About You:
Previous experience as a Care Manager, Case Manager, or Personal Services Agency Manager, required
Currently resides in the Indianapolis metro area, (or willing to relocate) with a deep commitment to serving the local community.
Willing to display a 100% commitment to the role, with a focus on providing consistent and reliable care management services.
Spanish speaking, preferred
CPR certified, preferred
Previous experience as a CPR Instructor, preferred
