Admissions Coordinator - Skilled Nursing Facility

What you will do in this role: Officially we title this job an Admissions Coordinator, but in other centers it may be called an Admissions Rep, Intake Coordinator or Admissions Liaison.When you are a patient, the transition from hospital to SNF is stressful.  As an Admissions Coordinator, you will help to organize multidisciplinary services so the patient/resident can safely transition into our center.  This could involve coordinating communication with the hospital medical staff, our internal clinical team and balancing insurance needs.  You will juggle many things at once:  One moment you may be analyzing the cost of meds, the next could be touring a family member, and the next is ensuring your intake packet is correctly signed. You will do whatever is needed to help us build our census and meet the needs of the patient.What’s in it for you:  Your opinion matters here – our administrator will ask for your inputFlexible scheduling – we understand emergencies come upEmployee recognition programAnnual merit increase opportunitiesRobust benefits package, including a retirement savings planRefer-a-Friend programs – Bring your friends & earn a bonus $.
