Supported Living Direct Support Professional

Responsibilities:

Provide direct care and support to individuals with disabilities in a supported living setting.

Assist clients with activities of daily living (ADLs) including personal hygiene, meal preparation, medication administration, and household chores.

Implement individualized support plans developed by supervisors and/or interdisciplinary teams.

Foster independence and community integration by promoting skill development and participation in social, recreational, and vocational activities.

Ensure the health, safety, and well-being of clients through attentive supervision and responsive intervention.

Document client progress, incidents, and observations accurately and in a timely manner.

Communicate effectively with clients, their families, coworkers, and other stakeholders to ensure coordinated care and positive outcomes.

Collaborate with healthcare professionals, case managers, and other service providers to address clients' holistic needs and achieve goals.

Participate in staff meetings, training sessions, and professional development activities to enhance knowledge and skills.

Adhere to organizational policies, procedures, and regulations, including maintaining confidentiality and respecting clients' rights and dignity.

Qualifications:

High school diploma or equivalent.

Previous experience working with individuals with disabilities, preferably in a supported living or similar setting.

Demonstrated understanding of person-centered planning and principles of empowerment.

Ability to communicate effectively and compassionately with individuals with diverse needs and backgrounds.

Strong organizational skills and attention to detail, with the ability to multitask and prioritize responsibilities.

Flexibility to adapt to changing environments and respond to emergent situations calmly and effectively.

Basic computer skills for documentation and communication purposes.

Valid driver's license and reliable transportation.

Ability to pass background checks and screening.
