If you are interested in this position, you will be required to confirm eligibility.The Care Manager, Registered Nurse, RN, Intern will utilize Humana's Medicaid training and engage formerly acquired skillsets to assess and evaluate members' needs and determine necessities to achieve and/or maintain optimal wellness. Work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action.
The Care Manager, Telephonic Nurse 2 employs a variety of strategies, approaches and techniques to manage a member's physical, environmental and psycho-social health issues with adult and/or pediatric population.
Identifies and resolves barriers that hinder effective care.
Ensures patient is progressing towards desired outcomes by continuously monitoring patient care through assessments and/or evaluations.
May create member care plans.
Understands department, segment, and organizational strategy and operating objectives, including their linkages to related areas.
Makes decisions regarding own work methods, occasionally in ambiguous situations, and requires minimal direction and receives guidance where needed.
Follows established guidelines/procedures.

Transitioning Service Member eligible to participate in SkillBridge or military spouse eligible to participate in the military spouse fellowship
Active unrestricted Licensed Registered Nurse (RN) in the state of residence or compact (eNLC) state license without disciplinary action
Clinical acute care experience in hospital floor, skilled, or rehabilitation setting
Comprehensive knowledge using Microsoft Office Word, Excel, PowerPoint, navigating multiple systems and platforms and ability to troubleshoot and resolve basic technical difficulties in a remote environment.

Possess an interest in working in the managed care industry
Experience with case management, discharge planning and patient education for adult acute care
Certified Case Manager (CCM)
Live in the state of Florida
