The role you’ll contribute:The Care Management Supervisor is under the general leadership of the Care Management Manager or Facility Director and assists inmonitoring and performing the daily operations and supervision of team members in the Care Management department.The Care Management Supervisor is responsible for the daily front line operations as assigned by the Care Management Manager orFacility Director. The Care Management Supervisor will serve as the primary point of contact for Care Managers, Social Workers andpatients, to provide mitigation for any barriers/concerns that may arise on the inpatient units. The Care Management Supervisor willbe responsible for supporting positive and safe patient outcomes, employee evaluations, and customer experience.The value you’ll bring to the team:Supervises daily operations of discharge planning and social services in the inpatient and emergency department setting as directedby Care Management Manager or Facility Director, including:Direct supervision of Care Management staffPrimary point of contact for inpatient unit Care Management related barriers/concernsManages own assignment and patient/department outcomes of those under direct supervisionAssists Care Management Manager/Facility Director to implement the Corporate Care Management strategiesEnsures active staff participation in daily Multidisciplinary RoundsServes as a point of contact for all identified stakeholdersServes as a content resource regarding medical necessity criteria, patient status and discharge criteriaAssures staff documentation is completed per guidelines as evidenced by appropriate monitoring techniques and coaches staff asneededFacilitates care conferences for complex transitions and/or placements, as neededParticipates in hospital/medical staff meetings to review patients meeting criteria related to high dollar/ extended length of stayand/or difficult placement seeks assistance from Care Management Manager/Facility Director, as neededReports pertinent quality/risk issues to appropriate individuals, departments and/or committees in a timely mannerDiscusses patient, staff, physician, and other department complaints with Care Management Manager/Facility Director and followsup with a written report submitted within 24 hours, as neededDemonstrates a positive problem-solving attitude to staff, physicians and peersServes as a supportive change agent for Care Management model processesActs as a role model for Care Management department staffEncourages team member engagementMaintains open communication with staffAssists with staff performance reviews/evaluationsProvides support and oversight with progressive corrective action plansBalances schedules to meet department needs prior to postingCommunicates changes to expected schedule minimizes unscheduled absencesPlans job responsibilities for maximum utilization of staffResolves daily staffing issuesMinimum qualifications :Registered Nurse: Associates Degree Nursing OR Social Worker: BSWTwo (2) years of experience in direct Care ManagementState specific RN license if a nursePreferred qualifications:Registered Nurse: BSNSocial Worker: MSWThree (3) years of experience in direct Care ManagementCCM, ACM, or RN-BC in Care ManagementThis facility is an equal opportunity employer and complies with federal, state and local anti-discrimination laws, regulations and ordinances.
