RN Case ManagerJob DescriptionThe RN Case Manager performs telephonic case management for Medicare Advantage members in the state of Florida. The case manager will be responsible for performing care coordination activities, developing care plans and assessments, conducting new referrals, calling members, and building new cases in Athena (EMR).Responsibilities

Essential Skills
RN license (FL license required)
2 years of case management experience from hospital/acute care/home health/hospice
Experience with Athena EMR software/platform
Strong computer skills: Microsoft Suites, Excel, data entry, and computer documentation
Ability to handle a caseload of at least 73

Prior experience in a fast-paced/high acuity nursing environment
Work EnvironmentThis is a hybrid opportunity with working hours from Monday to Friday, 8am to 5pm. Candidates must have a genuine desire to work in case management because they enjoy the work itself.Pay and BenefitsThe pay range for this position is $35.00 - $45.00/hr.Eligibility requirements apply to some benefits and may depend on your job classification and length of employment.
