The Community Health Worker Specialists work with the community, healthcare partners, and schools to improve systems of care and promote bi-directional communication between our local communities and the Department of Health Services. The Community Health Worker Specialists will provide information about programs and services offered by the Sonoma County Department of Health Services, assist with navigation of complex healthcare systems, and advocate for the needs and perspectives of local communities in Sonoma County. Community Health Worker Specialists will:
Conduct initial health assessments, promote safety net services, and help reduce incidences of chronic disease
Provide culturally appropriate health education and information
Engage with community members through health coaching and social support
Administer outreach through home visits and presentations at local agencies and community events
Advocate for the needs and perspectives of underrepresented populations in the community
Promote equitable access to health care through Health System Navigation
As an ideal candidate to join our team, you will have professional and/or lived experience with community members of historically underrepresented communities. Additionally, you will have:
The ability to work outdoors and engage in neighborhood outreach
Previous experience with stakeholders and agency partners to create support for community-based solutions in order to address health disparities
Knowledge of local health and social service systems and resources and the ability to navigate them effectively
The ability to speak easily with diverse community members, including clients, community partners, clinical staff, and County employees
Experience engaging in community outreach, particularly with low-income populations, individuals facing mental health difficulties, or those with medical fragility
Knowledge of basic public health principles, including how social factors and physical surroundings create conditions that can support or degrade health and an understanding of how differences can create disparities in the health of populations and across neighborhoods geographically
Experience facilitating health screenings for community members
Confidence in presenting and/or public speaking is desirable
