DESCRIPTION:

The Clinical Intake Specialist position provides support to youth and families who are seeking services from the agency. This position entails a wide variety of duties which includes being a primary access point for individuals in the community seeking services, scheduling intakes, gathering demographic and clinical information, coordinating care and documenting services provided, and providing exceptional customer service. The Clinical Intake Specialist position supports with clerical, analytic and other duties, assisting the Clinical Practice Manager and program staff and serves as a representative of the Quality Improvement Department. Supports the Mission, Visions, and Values of the agency.

QUALIFICATIONS:

Minimum of a high school diploma required with two years of experience in an administrative, support services role, or equivalent

Position Specific

Greet and orient families seeking services on referral and intake process

Collaborate with Programs Department in assisting access to care for new and potential clients

Gather and input pertinent demographic information, determining eligibility criteria, and maintaining tracking and scheduling processes for new and potential clients

Assist program staff with scheduling appointments and coordination of call & walk-in clients

Assist with site specific inventories, room reservations and lobby operations

Accurately enter data into internal and county electronic health databases

Accurately document services provided in the internal and/or county electronic health database

Answers multi-line phone system according to agency guidelines, in a welcoming and professional manner directing calls accordingly

Composes, types and edits correspondence, reports, memoranda and other material

Process referrals and ensure program access to referrals

Scan, copy, and input documentation into electronic record

Able to work a flexible schedule for walk-in clinics held at multiple sites

Develops and maintains department forms and procedures

Strong problem-solving skills, de-escalation techniques as needed

Cross trained to provide back-up support to other Quality Improvement staff as needed

Perform other duties to support the overall agency as needed

Model and communicate appropriate positive attitudes toward the agency’s Mission, Vision, and Values

Work collaboratively with all agency programs and staff to provide support as needed

Utilize and maintain calendar with all work-related details in order to manage time effectively and share calendar information with coworkers

Participate in on-going training to expand and develop professional skills

Perform other duties as necessary for the agency, as assigned

Employment At-Will

Employment at the Agency is terminable at-will, which means that employment may be terminated at any time, without cause or reason, by either the employee or the Agency. In addition the Agency may also demote, layoff, transfer or reassign employees at any time at its

sole discretion without cause or reason.

Starting Salary Range: $20.00 - $24.00 DOE
