Job Summary:
Private Care Management Company is looking for a dedicated and experienced Licensed Vocational Nurse (LVN) to provide personalized support to seniors in a flexible, part-time role. This position is ideal for an LVN seeking a change from traditional long shifts and looking to make a meaningful impact in the lives of elderly individuals. The role combines in-home and facility visits with office-based administrative work, ensuring clients receive comprehensive care. The ideal candidate is compassionate, detail-oriented, and interested in developing their skills in geriatric case management.

Responsibilities:
Conduct client visits in their homes or care facilities to assess health status and overall well-being and report concerns to care team.
Facilitate and accompany clients to doctor’s appointments as needed.
Assist with medication management, including setup and adherence monitoring.
Schedule medical and other necessary appointments for clients.
Coordinate with family members, caregivers, and healthcare professionals to ensure comprehensive care.
Maintain accurate documentation of client visits and assessments.
Perform office tasks as needed to support case management functions.
Communicate regularly with the care team and loved ones to ensure the highest quality of care.
Complete administrative tasks in the office when not in the field to assist with case management operations.
Requirements:
Valid LVN license in the state of California.
At least 8 years of LVN experience.
Experience in geriatric care or a related field preferred.
Reliable transportation, a valid driver’s license, good driving record.
Adequate computer skills to maintain client records. Apple preferred.
Ability to travel throughout the SFV and Westside areas.
Strong communication and organizational skills.
Compassionate and patient-centered approach to providing care.
Interest in transitioning into geriatric case management.
Schedule & Compensation:
Part-time, flexible schedule based on client needs.
Paid Mileage
Competitive hourly pay based on experience.
