We are seeking a Part-Time Case Manager to support our recuperative care program for individuals transitioning from hospital care. The ideal candidate will have experience working with hospitals, social services, and vulnerable populations.
Key Responsibilities:
Referral Management: Work directly with hospitals to review patient referrals and ensure they meet program criteria.
Admissions & Paperwork: Facilitate the intake process, including documentation and necessary paperwork.
Care Coordination: Assist patients in scheduling primary care appointments and accessing necessary healthcare services.
Housing Support: Help patients identify and apply for stable housing options while they are in our program.
Advocacy & Support: Work with community partners and social service agencies to connect patients with long-term support.
Qualifications:
Experience in case management, social work, healthcare navigation, or a related field.
Familiarity with hospital discharge processes and social services.
Strong organizational and communication skills.
Ability to work independently and handle multiple cases.
Schedule & Compensation:
Part-time position (hours flexible based on needs).
