Westside Family Health Center (WFHC) is a nonprofit community clinic dedicated to providing high quality,

The Patient Care Coordinator will provide in-reach and case management services to patients who are enrolled
in Medi-Cal and Medicare programs helping them navigate through health care services and systems. Promoting
the importance of wellness. The patient Care Coordinator will work closely with the patient care teams to support
positive health outcomes. The Patient Care Coordinator is the main point of contact for patients and removing
the patient’s barriers to care by assisting them to make appointments for wellness visits.
Key Responsibilities:
Must be well versed on all WFHC services.
Track patient information, schedules, files, and forms in a confidential manner
Meet with Chief Operating Officer and Chief Medical Officer to discuss in-reach issues and needs and
facilitate patient appointments.
Maintain documentation of all patient encounters and complete reporting requirements
according to organization standards
Be liaison for patients and team members with health insurance plans program
Submit data and tracking reports
Advocating to promote health care accessibility
Good communication and interpersonal skills and ability to speak concisely to patients and Care
Team member
Organized with confidential patient material and appointment tracking
Flexible and adaptable in response to changing patient and health care providers’ needs
Qualifications:
Two or more years of experience in customer service / sales capacity or one/or more years of
experience in a healthcare setting and prior experience in a health plan, medical group or IPA setting
working with physician networks
Desire to work with individuals from diverse ethnic, educational and socioeconomic backgrounds
Computer knowledge including ability to work with Microsoft Office, database and patient
management systems
Critical thinking and problem-solving skills
Effective verbal and written communication skills; strong computer skills required.
Self-motivated, flexible and a team player.
Bilingual English/Spanish is preferred.
Education: College or other post-secondary education preferred or work equivalent experience.
