This position is responsible for day-to-day operations of the Memory Care Communities. Expectations are to coordinate and implement programs and services designed for residents with cognitive impairment; meet or exceed standards of care; encourage and assist each Memory Care resident to function at their highest level of ability; and by word and action reassure families and friends that residents are safe secure and well cared for.
Responsibilities:
Communicate with community healthcare professionals, including physicians, discharge planners, nurses, physicians, pharmacy and hospice personnel and state agencies on the care needs of residents as needed.
coordinating incidental medical, dental, vision, hearing and podiatry care for residents and arrange resident escorts as needed.
Provide input to Director of Care Communicate with community healthcare professionals, including physicians, pharmacy, hospice, nurses and ancillary providers.
Provide input regarding resident pre-admission assessments to the Director of Care and or Administrator
Provide care staff with copies of the most current resident service plans and staff tasks sheets and monitor to ensure services are provided and documented according to company policy.
Interview potential residents and families and obtain medical records and other requirements as outlined in Title 22 for an admission to the community.
Observe residents and collect data pertinent to resident care on an ongoing basis.
Assist the Director of Care in managing staffing needs of communities
Works with team members to ensure 1) Medication administration records are correct 2) Medications are documented correctly 3) Medications are ordered timely 4) Adequate care items are in stock
Complete thorough documentation in resident chart on an ongoing basis according to company policy and state regulations.
Foster a resident-centered caring environment, making certain to improve the quality of lives for our residents
Directs, coordinates, and monitors nursing care delivery to assure safe, effective, and appropriate care
Works with other team members to monitor day to day care levels of residents for quality assurance and to verify appropriate levels of care as established
Maintains and oversees the assessment of comprehensive nursing needs of each resident
Establishes and implements quality assurance program for improved resident care, and quality of life programs according to company and federal policies
Evaluates performance of and supervises care associates
Requirements:
Nursing license in good standing
Must be an LVN
CPR/First Aid Certification
