Salida Del Sol Adult Day Health Care (ADHC)is a Community Based Adult Services (CBAS) licensed program that provides services to eligible older adults and/or adults with disabilities to restore or maintain their optimal capacity for self-care and delay or prevent inappropriate or personally undesirable institutionalization.
Job Description:
- Providing observation and monitoring of participant ’s health status.
- Providing personal care services for the participants as required by the individual plan of care.
- Assists program RNs in directing/overseeing staff on daily basis, to ensure patient safety.
- Conduct nursing assessments, monitor health problems of center participants, and create nursing care plans under RN supervision;
- Provides case-management on assigned participant enrolled in the ADHC Program.
- Works as a member of the ADHC interdisciplinary team, and contributes in developing and implementing the participant 's treatment plan of care.
- Routinely completes encounters and detailed, current, and comprehensive documentation of all provided services.
- Documentation of the time and dosage of each medication administered to a participant.
- Providing ERS participants telehealth weekly.
- Performs other duties as assigned depending on demands and/or needs within the Adult Day Health Care Program for continuity and coverage of Adult day health care.
Qualifications:
Valid CA LVN License.
Preferred Skill:
Bilingual in Spanish
