Description A local non-profit is seeking a dedicated and compassionate Case Manager to join their team. The Case Manager will be responsible for coordinating and providing comprehensive case management services to clients, ensuring they receive the necessary support and resources to achieve their individual goals. This role involves assessing client needs, developing personalized care plans, and collaborating with various service providers to deliver holistic care. This will be on-site in downtown Los Angeles Monday – Friday 8:30am – 4pm for an on-going contract. Responsibilities:

Conduct thorough assessments to determine client needs, strengths, and goals.

Complete intake processes, including documentation and initial case planning.

Develop individualized care plans based on client assessments.

Coordinate services and resources to support clients, including medical, social, educational, and employment services.

Monitor and adjust care plans as necessary to ensure they meet clients' evolving needs.

Advocate on behalf of clients to ensure they receive appropriate services and support.

Provide emotional support and guidance to clients throughout their care journey.

Educate clients and their families about available resources and services.

Work closely with healthcare providers, social workers, and other professionals to facilitate comprehensive care.

Maintain regular communication with clients, families, and service providers to ensure coordinated care.

Participate in multidisciplinary team meetings to discuss client progress and care strategies.

Maintain accurate and up-to-date case files and documentation.

Prepare and submit reports on client progress, outcomes, and any incidents.

Ensure compliance with all relevant policies, procedures, and regulations.

Respond to and manage crises as they arise, providing immediate support and intervention.

Develop and implement crisis management plans for high-risk clients.

Requirements

Qualifications:

Bachelor’s degree in Social Psychology, Human Services, Social Services, or related field) or associate's degree with a combination of relevant experience

Minimum of 2 years of experience in case management or a related field.

Case Management Certification (CCM) or eligibility to obtain certification is preferred.

CA Driver's license is required as driving may be necessary and valid automobile liability insurance

· If possible, the ability to deescalate volatile clients/situations-Non-profit experience -MS Office, Excel, Word Skills:

Strong assessment and analytical skills.

Excellent communication and interpersonal skills.

Ability to manage multiple priorities and work in a fast-paced environment.

Proficient in using case management software and other relevant technology.

Knowledge of community resources and social services
