LA’s leading agency serving the homeless population is looking for an Eviction Prevention Case Manager to join their busy and growing organization. Summary Under the direction of the Prevention Supervisor-Manager the Eviction Prevention Case Manager is responsible for screening and eligibility, service coordination, and eviction prevention for all participants with an eviction notice, 3-day notice, notice to pay or quit, or/and unlawful detainer. Essential Functions

Staff will be responsible for screening referrals to determine eligibility and coordinating client intakes and assessments.

Developing individual housing service plans, making linkages, and securing needed services for participants.

Meet with participants regularly or as needed to discuss progress and barriers.

Coordinating with other project staff and partners to provide housing placement services and supportive services to facilitate client’s successful stabilization in permanent housing and coordinate services to foster housing stability during and beyond eviction prevention subsidy.

Provide clients with post-discharge information and referrals for homeless prevention services and other ancillary services.

Staff will assist clients with identifying employment readiness; training and placement as needed or requested.

Coordinate ancillary services to achieve housing stability; follow-up with clients weekly, at minimum; maintain interagency consultation, coordination, and referrals as it relates to housing.

Maintain up-to-date and accurate documentation in client files for service coordination/case management, housing placement, rental arrears, rental assistance, and move in assistance.

Work collaboratively with the agency staff and LAHSA.

Provide diversion services if needed.

Provide the Senior Manager for Housing Services with all required client information and assessment outcomes.

Work collaboratively with outside agencies and partners.

Enter data into HMIS within 24 hours and enter all data in the system within 3 days of enrollment.

Prepare project reports in accordance with funding requirements.

Staff will assist in training and provide support to necessary staff, providers, and other project partners participating in project. The case manager will participate in all mandatory program and division meetings and trainings, as assigned by his/her supervisor and division director.

Prepare reports in accordance with program requirements and Division policies.

Conduct criminal background checks on all pre-eligible applicants and household members over 13 years of age.

Maintain and process all cases within 24-48 hours of program intakes.

Maintain files/records on client services in compliance with HIPAA, 42 CFR Part 2 and other funding requirements for audit purposes.

Engage and maintain appropriate communication with landlords.

Maintain appropriate boundaries; and adhere to the agency’s Code of Ethics and Core Values.

Represent the Agency in a professional manner at meetings and community events.

Secondary Functions

Perform other duties as assigned by the Senior Manager for Housing Services. Answer phones and route incoming calls; serve as back-up to the Intake Coordinator in his/her absence.

Experience, Education, Skills and Knowledge

Associate degree in social services or similar preferred.

1-2 years of job-related experience working with homeless individuals and families.

Knowledge of resource development, case management and documentation.

Ability to work with clients from diverse cultural, ethnic, and socio-economic backgrounds.

Working knowledge of Microsoft Word, Excel, and other database programs.

Experience using HMIS and at least one year of HPRP experience preferred.

Ability to communicate effectively, both written and orally.

Valid Driver’s License, reliable transportation, and auto insurance required.
