We have a Center CarePartner positions open with our PACE program, working at our Fortuna location. Our In-Home Carepartner is responsible for providing personal care services to participants in the Day Center and during transit to medical appointments according to departmental, para-professional, and regulatory standards of care. Must have a high school diploma or GED equivalent. Must have at least one (1) year of experience working with a frail or elderly population, preferably providing personal care in a health facility or in-home setting. Current CPR and First Aid certification required within six (6) months of hire (certification provided upon hire). Certified Nursing Assistant, Certified Home Health Aide, or Injections Certificate preferred (additional compensation may be available for certified individuals). This position is 32.5 hours per week, Monday-Friday, 8:15am-3:15pm.
This position qualifies for additional monetary incentives thru 09/2024
