We have In-Home CarePartner positions open with our PACE program, working at our participant's homes throughout Humboldt County. Our In-Home Carepartner is responsible for providing home care services to Redwood Coast PACE participants in their homes as authorized in the participant's Plan of Care as determined by the Interdisciplinary Team. Our opening is for both Full-time and Part-time shifts.
High school diploma or GED equivalent required. Minimum one (1) year of experience working with a frail or elderly population required, preferably providing personal care in a health facility or in-home setting. Current CPR and First Aid certification within six (6) months of hire. Valid California Driver’s License, and personal auto liability insurance required. Must have a personal vehicle in good driving/operating condition, insured for State minimum liability requirements, and that can be used for the applicable job functions noted above. Certified Nursing Assistant, Certified Home Health Aide, or Injections Certificate preferred (additional compensation may be available for certified individuals).
This position qualifies for additional monetary incentives thru 09/2024
