JOB TITLE: Case Manager

REPORTS TO: Reentry Center Director

SUPERVISES: n/a

DESCRIPTION STATUS: Non-Exempt

SALARY/HOURS: $20-25/hr.

HOURS: FT

LOCATION: 417 Barrow Sreet, Anchorage

Partners for Progress, Inc. is a Social Justice 501 (c)(3) non-profit organization seeking an organized multi-tasker, with a positive attitude, and strong communication and team-building skills. The Case Manager is a key position as part of the small team that operates the busy Partners Reentry Center in downtown Anchorage. Staff members work as a team with the goal of providing program and client support. This is accomplished in a manner that is welcoming and mutually respectful.

POSITION SUMMARY

Works directly with client base, performing intakes and longer-term case coordination. Maintains client files through electronic databases and provides real time case notes and input. Assesses client eligibility for various services offered in house. If ineligible for elements of, or our program in its entirety, refers to local agencies that can potentially meet immediate needs of client. Seeks out knowledge of community and statewide programs for the benefit of the client for service linking and educating. Provide support by connecting clients with necessary resources for successful community reintegration.

ESSENTIAL FUNCTIONS AND JOB DUTIES

1. Case Coordination

Responsible for reentry planning, coordination of services, coordination of data collection and reporting. Reports regularly to the Center Director and attends weekly case management with all providers. Supports and Mentors participants, recommend services and pro-social support groups.

Performs Intakes and Inputs required data into various required databases: AKAIMS and PFP Client database, on a real time basis.

Responsible for Case Load and Case Management for Partner’s Reentry. Provide Intake, Case Planning. Work with referral partners to implement and manage program. Responsible for regular/scheduled follow up and tracking of all clients for 1-6 mos.

Assure all relevant information pertaining to clients is released to all appropriate parties concerned in a timely fashion.

Assure all case notes and referrals are entered into required Database’s in real time, daily including State of Alaska AKAIMS.

Provide housing counseling and coordinate with Money Management for budget training for clients to help prepare them for independent housing.

With Partners Employment and Training Department, develop training and workshops for clients and the organization’s staff, including; tenant rights and responsibilities, and complying with transitional program regulations.

Confer with team on appropriate referrals for Mental Health, Addiction, Peer Support and MRT Services such as Peer2Peer and 12 Step Groups, Assessments, Public Assistance, School/GED Services, Vetrans, Dept. of Labor, AWS, DVR, TVR, and Emerg. Services.

Conduct Inreach to prisons as requested.

Collect information for data reporting.

Attend all required Case Management, All Staff and other meetings as needed

Attend trainings as requested by Management

Submit all required administrative reports, ie: travel, timesheet’s, leave requests, ect., In a timely fashion

Apply for various funding for participants, ie: Mental Health Trust Mini-Grants

Coordinates and supports weekly All Nations Reentry Group meetings and activities.

2. Other

Attend all required Case Manament, All Staff and other meetings as needed

Attend trainings as requested by Management

Submit all required administrative reports, ie: travel, timesheet’s, leave requests, ect., In a timely fashion

Helps to coordinate and support weekly Peer Support and Behavioral Health Groups, meetings and activities

REQUIRED ESSENTIAL JOB QUALIFICATIONS:

1. 2 yr. degree in Human Services or related field and 2 years relevant work experience.

2. 4 yr. degree in Human Services or related field Preferred. 2 years experience working with offender or high-risk populations preferred. Certificate or Accreditation in Substance Abuse Counseling Preferred

3. A combination of education and experience that demonstrates the ability to perform the essential functions of the position may be considered.

4. Ability to communicate in a positive, effective manner verbally, in writing, via email in the office environment and anytime representing PFP. Ability to work productively and cooperatively in an office environment. Ability to work as a team member is a positive, supportive way.

5. Ability to work with clients in a positive, respectful way

6. Good time management skills and punctuality, ability to meet deadlines.

7. Ability to manage work time and a flexible schedule.

8. Advanced knowledge and ability with Microsoft Programs including Powerpoint, Word, Excel, Access and Outlook. Intermediate knowledge of Google mail and docs.

9. Good organization skills and ability to coordinate multiple schedules.

10. Ability to keep professional boundaries (i.e. appropriate language for a professional office setting and excellent people skills).

11. Must demonstrate personal and interpersonal qualities that support the core values and mission of Partners for Progress.

12. Appropriate and acceptable behavior, hygiene and office attire required.

13. Must be able to pass a criminal background check or qualify for a variance if needed.
