Experience Needed:
Candidates will have successfully completed a Licensed Practical Nurse (LPN) curriculum, hold a license in good standing in Wisconsin, and have proven experience. The ideal candidate will possess a track record of professional, successful, and resident-focused work experience and attitude and will be patient, friendly, and have excellent communication skills.
Proven experience as an LPN and licensed (in good standing) in Wisconsin
Ability to undertake a variety of diverse care tasks and work in a multidisciplinary setting
Understanding of confidentiality obligations and nursing best practices
Excellent knowledge of emergency response
Excellent communication and interpersonal skills
Willingness to adhere to health and safety standards to protect the residents/patients
Respectful, compassionate, and able to multi-task
Excellent time management skills
Able to report and communicate resident information to Management timely and accurately
Strong ethics
Requirements:
State of WI LPN License, in good standing
Reliable Vehicle
Working Cell phone
Current TB Screening (within one year)
Willingness to work at multiple locations OR be dedicated to one specific location
What are the Job Duties of a FCMS CNA?
Assist residents with ambulation and mobility
Assist residents with personal care and hygiene
Help residents with physical therapy exercises, as needed
Assist with meal preparation, feeding, clean-up (varies by Client)
Housekeeping duties (varies by Client)
Be pleasant and supportive to the Residents
Report any unusual incidents
Accurately and timely charting as required by the Client
Act quickly and responsibly in cases of emergency
What are the Requirements of a FCMS CNA?
Must be 18 years old to apply
Proven experience as a CNA (prefer 1-2 years)
Current Wisconsin License of CNA Credentials
Working cell phone with voicemail
Excellent knowledge of emergency response and first aid
Knowledge of housekeeping tasks
Willingness to adhere to standard precautions when on client’s site
Respectful and compassionate
Excellent time management skills
Interpersonal skills
What are the Job Duties of a FCMS CBRF Caregiver?

Proven experience as a CBRF Caregiver (prefer 1-2 years)
Completion of all 4 CBRF Caregiver Training Courses and Credentials listed on the UWGB Registry

Excellent knowledge of emergency response and first aid
Knowledge of housekeeping tasks
Willingness to adhere to standard precautions when on client’s site

Our candidates must hold valid Wisconsin LPN license, CNA Certified Nursing Assistant curriculum and hold a current Wisconsin CNA License or completed the 4 CBRF Certification Credentials. The ideal candidate will also have a professional and compassionate attitude, be patient, friendly, have excellent communication skills, the ability to follow instructions and perform a variety of tasks to aid the residents/patients.
