Description:RN Care Manager is responsible for clinical assessment, member-centered care plan development, ongoing care coordination, quality assurance and monitoring of services to program members, who may by older adults and/or individuals who have intellectual or physical disabilities.

completes discipline-specific initial and ongoing comprehensive assessments
conducts home visits and home assessments as needed, and consults w/ families and other supports for additional information as needed
Uses critical thinking to optimize members' health and quality of life to reach goals
Develops and maintains care plans
