The C.N.A., in collaboration with patient, family, and health care team, will provide direct and indirect nursing care of the patient utilizing the nursing process under the supervision of a Registered Nurse according to the following work systems/functions.MINIMUM QUALIFICATIONS:
EDUCATION, CERTIFICATION, AND/OR LICENSURE:
1. High school graduate or G.E.D.
2. Certified Nursing Assistant (C.N.A.) certification
3. Valid driver's license.
4. CPR certification within 30 days of date of hire.CORE DUTIES AND RESPONSIBILITIES:The statements described hereare intendedto describe the general nature of work being performed by people assigned to this position. Provides personal and hygienic care every visit as per care plan and basic care standard in accordance with age-specific needs and policy and procedure.
Maintains a clean, safe, and comfortable environment per the basic care standard within the limitations of the home setting.
Ensuresprivacy and demonstrates concern for dignity of patientand respects patient's property in performing all patient care.
Completes assignments in a timely manner as per standard.
Follows plan of care assigned by RN.
Relays critical information to the appropriate staff regarding patient condition.
Confers with the primary nurse on the original plan of care and any subsequent updates.
Documents patient status and care of services furnished in appropriate areas as defined by the documentation policy and procedure.
Reports observed changes in patient condition to R.N.'s, including recognition and reporting of changes in level of comfort.
Reports abnormalities in vital signs to R.N.
Utilizes supplies effectively and to ensure cost containment.
Perform the following treatments/procedures according to the specific standards included but not limited to:
A. Measuring vital signs
B. Lifting and moving patients
C. Positioning the patient in bed
D. Transferring patient from bed to chair
E. Ambulating patient
F. ROM
G. Bed-making
H. Personal Hygiene
Oral care - brushing teeth, denture care
Bathing - bed bath, tub bath, shower
Back massage
Perineal Care
Hair Care - brushing and combing, shampooing in tub or shower, at sink, in bed
Shaving
Nail Care
Changing clothes
I. Assisting with elimination
Bed pan
Urinal
BSC
Ostomy care
J. Patients' weights
K. Case care

Frequent walking, standing, stooping, kneeling, reaching, pushing, pulling, lifting, and grasping are necessary body movements utilized in performing duties through the work shift.
Must be able to lift, move, and position patients of all weights, with varying amounts of assistance in a fair manner on a regular and recurring basis.
Hearing within normal range is required (e.g., apical pulse, monitors, etc.).
Visual acuity must be within normal range.
