Essential Functions:Performs discharge planning/Advance DirectivesDemonstrates the ability to plan, arrange and continually monitor caseload for discharge need via treatment plan with other disciplinesProvides consultations regarding patient/employee problemsMaintains and continues professional developmentPerforms other support activitiesCare is planned to respond to each patient's unique needs, including age-related needsDemonstrated competence in social workReports to: Director of Case ManagementMinimum Education
