Associates degree from an accredited school approved by the American Occupational Therapy Association.

Current license to practice in the state serviced.

Must obtain and maintain current CPR Certification

One-year occupational therapy experience required.

Must obtain a National Provider Identifier (NPI) as this is now a requirement in step with the Health Insurance Portability and Accountability Act (HIPAA)

Home care experience preferred.
