Together, we are empowered to make an even greater impact on the health and well-being of our communities.The CHI Franciscan Hospice team provides end of life care to 700 patients per day in a wide variety of settings including homes, Adult Family Homes, Nursing Homes, Hospice House and Assisted Livings. We provide patient centered care focusing on comfort and quality of life. Staff are organized into multiple small teams based on geography and whether patients live in facilities or homes. Franciscan Hospice reaches patients in the 4,000 square mile area including King, Pierce and Kitsap Counties. Staff need to be willing to travel within their assigned area.ResponsibilitiesRole:Hospice Social Worker as a member of a multi-disciplinary team, responsible for assessing and providing support and intervention for the social and emotional needs of patients and their families to promote continuity of care and services. Work involves professional activities such as psycho-social assessment, mental health evaluation and referral, crisis intervention, environmental assistance, patient advocacy, after-care planning and others of similar scope and complexity, depending on the assigned work unit.This job exists in multiple locations, assignments may include coverage at multiple locations and settings, and while there may be minor differences in job content, they are not significant for classification purposes. Overall, the nature of the work and job requirements are consistent between locations.QualificationsEducation & Work Experience requirements:Master’s Degree in Social Work requiredTwo years recent related work experience that would demonstrates attainment of the requisite job knowledge skills/abilities is preferred.Licensure & Certification:Current Licensed Independent Clinical Social Worker (LICSW)Current Licensed Advanced Social Worker (LASW)OrEligible for Agency Affiliated Counselor; and the state must receive the application within seven (7) days of employment; and the credential must be obtained within 60 days of hire.
