Orchard Crest Retirement is looking for a professional individual who is passionate about working with the elderly community.

The position we are looking to hire for is for a Health Services Assistant in our Assisted Living & Memory Care. This position reports directly to the Health Services Director and is integral in promoting the health and well-being of our residents, and communication with the Health Care Services Director and the nursing team.

Job responsibilities for the HSA include but are not limited to:

Create and complete the 14-day and 30-day assessments of new Assisted Living residents. Sign off upon completion.

Assist with the smooth intake and admission of new residents to Assisted Living programs. Be the designated "point person" on admission day.

Initiate and complete the reassessments of Assisted Living residents. This may include, but not limited to MMSE, skin, weight reviews, and other required components of the re­assessments process.

Proficient with all required documentation, this includes but not limited to MMSE, skin assessments, fall risk, event reports, and event investigations, MARS, family conferences, meetings, and calls.

Monitor all alert charting, D/C as needed and provide LPN with follow up on reports.

Responsible for maintaining Event Reports; providing LPN with injury events, providing the Administrator with completed events and copies of all event reports.

Responsible for monitoring family and self-provided medications. Follow up with families, ensure medications are accurate and alert in the case of inaccuracies, non-compliance, or breach of contract.

Ensure that medications are available by obtaining physicians orders and completing pharmacy requisitions and pharmacy contact.

Proficient with MARS process and monitors for accuracy.

Monitoring of health status of Assisted Living residents.

Consult with community health care providers regarding residents, as needed.

Function as a liaison with physicians, hospitals, families, ancillary health services, hospice, and private caregivers.

Physical Requirements:

· Must be able to cope with physical and mental stress.

· Must be able to perform essential job duties with or without reasonable accommodations, and not put others at risk.

· Must be able to lift, push, pull or carry a minimum of 20 pounds

Universal Precaution Risk:

· Tasks may involve contact exposure to blood/bodily fluids.

· Tasks may involve contact with communicable diseases.

Experience & Education Requirements:

(1) Has a master's degree in social services, human services, behavioral sciences or an allied field and two years social service experience working with adults who have functional or cognitive disabilities; or

(2) Has a bachelor's degree in social services, human services, behavioral sciences, or an allied field and three years social service experience working with adults who have functional or cognitive disabilities; or

(3) Has three years of successful experience acquired prior to September 1, 2004, assessing prospective and current assisted living facility residents in a setting licensed by a state agency for the care of vulnerable adults, such as a nursing home, assisted living facility, or adult family home, or a setting having a contract with a recognized social service agency for the provision of care to vulnerable adults, such as supported living.

Specific Requirements:

· Must be able to make independent decisions yet work within the scope of practice and function as a team member.

· Must have an active Washington State NAC/HCA License.

· Med-Tech experience preferred.

· Able to use E-MAR systems.

· Must be able to read and write legibly.

· Favorable background report.
