Northwest Regional Council (NWRC) is a multipurpose human services agency that relies on high performing employees to provide the best possible community-based supports for a variety of individuals. We are seeking applicants for our On-Call Care Coordinator position.

The role of Care Coordinator provides both intensive care coordination and comprehensive case management utilizing health screening/assessment tools, health education, and evidence-based practices to help clients establish health action plans. The Care Coordinator assists our highest-need clients, providing ongoing support to aid them in achieving established health and recovery goals.

On-Call status indicates this is a non-exempt, on-call, hourly position created specifically to address caseloads left open due to staff vacancies or other absences. The On-call position has fluctuating hours of fewer than 70 hours per month and is not eligible for our standard benefits package. Access to our Employee Assistance Program (EAP) and accrual of sick leave on a pro-rated basis are available.

10-STEP PAY RANGE: $29.28 - $38.20 hourly

QUALIFICATIONS:

An equivalent combination of education and experience that provides the skills, knowledge, and ability to perform the work may be considered.

Education/Experience:

Bachelor's Degree in behavioral or health sciences, or related field, and two years of experience providing direct human services to clients, preferably in a community setting OR

Master’s Degree in behavioral or health sciences, or related field, and one year of experience providing direct human services to clients, preferably in a community setting

Knowledge, Skills, and Abilities:

Demonstrated ability to assess client health and functional status, understand and develop appropriate recovery plans and make clear and concise recommendations;

Demonstrated advocacy skills and sensitivity to the needs and values of diverse groups;

Knowledge of the local providers, services and resources, as well as the formal and informal support networks available in the service area a plus;

Skill and/or fluency in speaking Spanish or Russian a plus.

Licenses/Certificates/Special Requirements:

Must have a valid Washington State driver's license, a vehicle available for work-related travel, and appropriate liability insurance.
