The Memory Care Social Worker and Activities Coordinator is a unique role, which involves performing the typical duties of a social worker in a skilled nursing facility along with directing and facilitating the activity program on the unit. This unit is smaller than most typical skilled nursing facility units and rather than have a different part-time employee in each role, we find that the ability to engage the residents in activities provides an excellent foundation for the relationship-building, conducting of assessments and creating of interventions that a social worker is responsible for.
Responsibilities of the role include, but are not limited to the following:
-Conducting of psychosocial, mood, cognitive and behavior assessments
-Collaborating with an interdisciplinary team
-Facilitating family care conferences
-Developing person-centered interventions for mood and behavior
-Facilitating group, individual and 1:1 activities in the memory care unit
-Documenting both social work and activity notes and assessments in an electronic medical record
-Building relationships with families
Minimum Requirements:
-Initial Covid-19 vaccination
-A Bachelors Degree
-Strong verbal and written communication skills along with customer service skills
-Time management and organizational skills
-Computer skills for charting and documentation
-A strong team player
-Ability to work autonomously, with support from your team and supervisor
-Patience
-A desire to work with elders with dementia
